What Are Breath-Holding Spells?

Breath-holding spells are brief episodes, most common in children between about 6 months and 6 years of age, where a child involuntarily stops breathing for a short period, usually triggered by crying, pain, fright, or frustration. Despite how frightening they look to parents, they are generally benign and not within the child's conscious control — the child is not "holding their breath on purpose" to manipulate a situation.

What a Spell Looks Like

A typical episode starts with crying (sometimes just a brief cry or even silently), followed by the child stopping breathing, changing colour, and sometimes becoming briefly limp or losing consciousness for a few seconds. In some cases, a few jerking movements can occur as the child recovers, which can look alarming but is part of the body's normal response to a brief dip in oxygen or blood flow, not a seizure disorder.

Why They Happen

Breath-holding spells are thought to result from an exaggerated reflex response, often described as involving the autonomic nervous system's regulation of breathing and heart rate, rather than any deliberate action by the child. They tend to run in families, and a child who has one spell often goes on to have further episodes with similar triggers until they grow out of the tendency.

Cyanotic vs Pallid Breath-Holding Spells

  • Cyanotic spells (more common): usually triggered by anger, frustration, or being told "no"; the child cries, then turns bluish around the lips, and may briefly pass out
  • Pallid spells (less common): usually triggered by a sudden fright, pain, or a bump to the head; the child may make little or no sound, turn pale, and lose consciousness briefly, sometimes with a brief stiffening or a few jerks

Most children outgrow breath-holding spells by around 4 to 6 years of age, and the episodes generally do not cause any lasting harm or affect long-term development. Spells typically become less frequent over time even before they stop completely.
