What Is Hand, Foot and Mouth Disease?

Hand, Foot and Mouth Disease (HFMD) is a common, contagious viral illness that mainly affects infants and young children, especially those in daycare or playschool settings where close contact is frequent. Despite the name sounding alarming, it is usually a mild illness that resolves on its own within seven to ten days with supportive care at home.

  • Mild fever, often the first sign
  • Painful mouth sores, which can make eating and drinking uncomfortable
  • A rash or small blisters on the hands, feet, and sometimes the buttocks and knees
  • Reduced appetite and general irritability, largely due to mouth discomfort
  • Sore throat

Not every child shows the full pattern. Some have mostly mouth sores with little rash, while others show a more visible rash with milder mouth symptoms.

How It Spreads

HFMD spreads easily through close contact, respiratory droplets, and contact with fluid from blisters or with stool, which is why outbreaks are common in daycare and playschool groups. Good hand hygiene, for both children and caregivers, along with regular cleaning of shared toys and surfaces, is the most effective way to reduce spread within a household or classroom.

Caring for a Child at Home

  • Offer cool fluids and soft, bland foods that are easier on a sore mouth
  • Avoid acidic, spicy, or salty foods that can sting mouth sores
  • Use fever-reducing medicine as appropriate for comfort, following your pediatrician's dosing guidance
  • Keep an eye on fluid intake, since painful swallowing can sometimes lead to reduced hydration, particularly in younger toddlers
  • Allow extra rest, as the body recovers over several days

When to Call a Pediatrician

Most cases of HFMD are managed comfortably at home, but contact a pediatrician if:

  • Your child shows signs of dehydration, such as noticeably fewer wet diapers, dry mouth, or unusual drowsiness
  • Fever is high or lasts more than a few days
  • Your child refuses most fluids because of mouth pain
  • Symptoms seem unusually severe or are not improving after a week
  • A child under six months develops similar symptoms

When Is It Safe to Return to Daycare?

Most schools and daycare centres ask that a child stay home while fever is present and until mouth sores and blisters have started to dry up or heal, generally once the child feels well enough to participate normally and is fever-free. Since the virus can still be present in stool for some time after symptoms resolve, continued good hand hygiene remains important even after your child returns.

Because outbreaks in daycare settings are common, many schools send home a general notice when a case is identified in the class, without naming the affected child, so other parents can watch for early symptoms. If your child's school or playschool has an active case, extra attention to handwashing before and after school, and avoiding sharing of cups, utensils, and toys that go in the mouth, can help reduce the chance of your own child catching it.
