Ovarian Cysts: Symptoms, Types, and When They Need Treatment
What Is an Ovarian Cyst?
An ovarian cyst is a fluid-filled sac that forms on or inside an ovary. They are extremely common, and most women will develop at least one ovarian cyst at some point in their reproductive years, often without ever knowing it, because many cysts cause no symptoms and resolve on their own.
Common Types of Ovarian Cysts
- Functional cysts (follicular and corpus luteum cysts): the most common type, forming as a normal part of the menstrual cycle, and usually resolving on their own within a few weeks to months
- Dermoid cysts: develop from cells present from birth and can contain tissue such as hair or fat; they don't resolve on their own and are usually monitored or removed
- Endometriomas: cysts that form when endometriosis tissue grows on the ovary
- Cystadenomas: cysts that develop from ovarian tissue and can grow fairly large
Symptoms to Watch For
Many ovarian cysts cause no symptoms at all and are found incidentally during a routine scan. When symptoms do occur, they can include pelvic pain or pressure (often on one side), bloating, a feeling of fullness, pain during intercourse, or changes in the menstrual cycle. Larger cysts are more likely to cause noticeable symptoms than small ones.
When Ovarian Cysts Need Attention Urgently
Occasionally, a cyst can twist (ovarian torsion) or rupture, both of which cause sudden, severe pelvic pain, sometimes with nausea, vomiting, or fever. These situations need urgent medical evaluation, as prompt treatment can help preserve the ovary.
How Ovarian Cysts Are Diagnosed
An ultrasound scan is the main tool used to identify an ovarian cyst, assess its size, and get a sense of whether it looks like a simple functional cyst or something that needs closer monitoring. Depending on the findings, a gynaecologist may also recommend blood tests or repeat imaging after a few weeks to see if the cyst has resolved on its own.
Treatment Options
Most functional cysts simply need watchful waiting, with a follow-up scan to confirm they have resolved. For cysts that are large, persistent, growing, or causing significant symptoms, options range from hormonal management to reduce new cyst formation, to surgical removal (often done laparoscopically) when a cyst needs to come out — for example, if it is very large, suspicious in appearance, or causing ongoing pain.
Ovarian Cysts and Fertility
Most simple ovarian cysts do not affect fertility. Certain types, such as endometriomas, or situations requiring surgery, can have some impact on ovarian reserve, which is why a gynaecologist will weigh the benefits and risks of any intervention carefully, especially in women who wish to conceive in the future.
When to See a Gynaecologist
See a gynaecologist if you have persistent pelvic pain, noticeable bloating that doesn't settle, changes in your periods, or if a cyst has been found on a scan and you want to understand what it means and what, if anything, needs to be done. Seek urgent care for sudden, severe one-sided pelvic pain, especially with vomiting or fever.
Frequently Asked Questions
Do all ovarian cysts need surgery? No. Most functional cysts resolve on their own within a few weeks to months and simply need monitoring. Surgery is usually reserved for cysts that are large, persistent, suspicious in appearance, or causing significant symptoms.
Can ovarian cysts affect my ability to conceive? Most simple, common cysts do not affect fertility. Some types, such as endometriomas, or surgery to remove a cyst, can potentially affect ovarian reserve, so this is discussed individually with your gynaecologist.
Are ovarian cysts related to PCOS? PCOS involves multiple small follicles on the ovaries along with hormonal changes, which is a different pattern from a single functional or other type of cyst, though both fall under conditions a gynaecologist evaluates with ultrasound and hormone testing.
What symptoms mean I should seek care urgently? Sudden, severe pelvic pain — especially on one side — along with nausea, vomiting, or fever can indicate a cyst has twisted or ruptured, which needs prompt medical evaluation.
